Final Self-Evaluation (due 12/9)

Overview

• The Final Self-Evaluation form is designed to help you evaluate your progress in a number of specific focus areas as well as complete a final reflection on the development of skills, integration of knowledge, clarity of your calling, and use of resources throughout your internship.
• The Final Self-Evaluation form is completed by you. Signatures of supervisor and guidance committee are recommended but not required. Self-evaluations are not shared with anyone unless released in writing by you. 
• The Final Self-Evaluation form must be completed no later than Monday, 12/9/24.

Instructions

• The Final Self-Evaluation form is to be completed by you alone. It is important to note that your site supervisor and your guidance committee will be evaluating you using many of the same questions plus some specific to their observations/roles. It is recommended but not required to collect signatures from your site supervisor and guidance committee chairperson, but it can be useful for discussion to share it with them. 
• To complete the Final Self-Evaluation form, download the attached Word document, complete it, and upload a copy to this assignment dropbox.
• Optionally, you may include any additional attachments or comments that may supplement your final self-evaluation and reflection, if you choose to do so. 
• The Final Self-Evaluation form must be submitted no later than Monday, 12/9/24. Please note that a late or missing form could result in an internship grade of Fail. 

Completion

• Upon receiving the completed form, Dr. Ward will post feedback or comments. Feedback will be provided within 48 hours of receipt, whenever possible. 
• The Final Self-Evaluation form must be submitted no later than 12/9/24. Please submit an extension request if you cannot meet this deadline. Failure to submit an extension request form by 12/9/24 will result in an internship grade of Fail.
