Overview
- The Final Guidance Committee Evaluation form requests a summative evaluation of your skills in specific focus areas, temperament and sense of call, and progress on established internship goals.
- This form should be completed by the guidance committee chairperson, in consultation with the guidance committee as a whole, and submitted no later than Friday, 7/12/24.
Instructions
- The Final Guidance Committee Evaluation is to be completed by the guidance committee chairperson, in consultation with the guidance committee as a whole. It is highly encouraged that you set up a time to meet with your guidance committee to discuss the items in the evaluation before the form is submitted.
- Students are responsible for providing a copy of the attached document of the Final Guidance Committee Evaluation form to the guidance committee chairperson via email.
- To share the form, download the attached document, attach it to an email, and send it to your guidance committee chairperson, or print and provide them a physical copy. Guidance committee chairpersons may send the completed evaluation back to you or directly to Dr. Ward, lward@dbq.edu or submit it back to you to submit in this assignment dropbox.
- The Final Guidance Committee Evaluation form must be submitted no later than Friday, 7/12/24. Please note that a late or missing form could result in an internship grade of Fail.
Completion
- Upon receiving the completed form, Dr. Ward will attach a copy to this assignment dropbox with any necessary comments. Feedback will be provided wihtin 48 hours of receipt, whenever possible.
- The Final Guidance Committee Evaluation form must be submitted no later than 7/12/24. Please submit an extension request if you cannot meet this deadline. Failure to submit an extension request or form by 7/12/24 will result in an internship grade of Fail.
