Overview
- All SPM sites must be approved prior to students beginning their internship.
- Please follow the instructions below to receive site approval. Approval must be requested no later than Friday, 5/24/24.
Instructions
- To request approval for your site, students must submit an SPM Approval Form to this assignment link. Please note for Summer 2024, no electronic signature platform is available for this form. It will need to be completed as described below.
- To request site approval, please include all information requested on the form attached below and attain all requested signatures. Signatures can be attained in one of two ways:
- Type and print the attached document, receive physical signatures, and scan/take photos of the document, then submit to this link.
- Type and save the attached document and send via email to proposed supervisor and guidance committee chairperson requesting email confirmation as "signatures." Then forward the email chain to Dr. Ward at LWard@dbq.edu.
- If the document is scanned, please upload and submit to this assignment link. If emailed, no submission to the assignment link is necessary.
- While the approval deadline is 5/24, ideally, all sites are approved prior to that date, so early submission of the request is highly encouraged.
Completion
- You will receive approval notification of your proposed site through this assignment link within 48 hours of submission, whenever possible.
- If your site is approved, you may move forward into Step 2: Learning Covenant.
- If your site is denied, you will need to update approval with missing criteria, locate a new site, or withdraw from the credit and enroll in another term.
- Please complete the approval process no later than Friday, 5/24/24.
