    The purpose of this page is to provide cooperating and supervising teachers with the resources and dates needed to meet the reporting requirements of student teaching.

  • Supervising Teachers

    • Bi-weekly formal Observations with feedback--using common observation form--this is a MINIMUM requirement 
    • Progress reporting (4x throughout semester)--can be done alone or in collaboration with cooperating teacher 
    • Observation log 
    • Collaboration with cooperating teacher on final grade
    • Completion of ONLINE final evaluation document--can be done alone or in collaboration with cooperating teacher 

    Cooperating Teachers

    • Ongoing verbal/written feedback for student teacher--journals/notebooks are a great way to accomplish this
    • Progress reporting (4x throughout semester)--can be done alone or in collaboration with supervising teacher 
    • Collaboration with supervising teacher on final grade
    • Completion of ONLINE final evaluation document--can be done alone or in collaboration with supervising teacher 

    Other (Cooperating and Supervising Teachers)

    • Ongoing mentoring and support of student teacher
    • Communication with student teaching supervisor
    • Feedback on lesson plans
    • Timely completion of reporting

    Final Eight Weeks--Select Supervising and Cooperating Teachers (only those who received an email)

    • Supervising teacher completes four formal observations
    • Cooperating teacher completes two formal observations
    • Collaborative progress reporting 
    • Collaborative final grade and final formal (online) evaluation
    • Other duties aligned in sections relevant sections above


  • Eight Week Placements

    • January 2-February 27 (Ends on a Tuesday)--36 Student Contact Days
    • February 28-April 25 (Begins on a Wednesday)--35 Student Contact Days

      This form is for all formal observations (completed every 2 weeks by the supervising teacher). Please complete electronically and return to Dr. Haverland via email.

      Supervising Teacher Observation Log URL

      This running form should be completed for each visit between the supervising teacher and student teacher and emailed back to Dr. Haverland at the end of each placement.

      Progress Report URL

      This form is completed by the supervising and cooperating teachers either separately or collaboratively (2 times for each 8 week placement or 4 times for each 16 week placement) and returned to Dr. Haverland via email.

      Formal Online Evaluation Link (Microsoft Forms) URL

      Please use this link to complete the final evaluation of the student teacher for each placement. This should be completed by both the cooperating and supervising teacher at the end of each placement. That said, it can be done collaboratively. This is a substantial form to evaluate progress on character dispositions and InTASC standards, and it does not allow you to come back to complete it. 

      To complete this evaluation, you will need the following information:

      Final Student Teaching Grade URL

      This form is used to assign a final grade for each student teaching placement and should be completed collaboratively and emailed to Dr. Haverland.

    • Mock Evaluation

      There is a requirement in the state of Iowa that student teachers participate in a mock evaluation of their teaching. This evaluation can be done by an administrator, cooperating teacher, instructional coach, or designee. There are two forms that follow. The first is the form stating that the evaluation was completed. This form must be signed and returned to me. The second form is an evaluation instrument based on the Iowa Teaching Standards. This form is to help facilitate the evaluation (or you can use the tool of your choice) and you are not required to return anything to me other than the signed form.

      This form must be completed by your student teacher following their mock evaluation and returned to me. It is merely for your information.

      Mock Evaluation Form--Iowa Teaching Standards URL

      This form can be used by you, your administrator, or your designee to evaluate student progress according to the Iowa Teaching Standards. This form does not need to be returned to me. 

    • School-Based Interview

      An important part of the student teaching placement is for students to have an opportunity to participate in an interview (for a real or imagined position) with a school administrator. The hope is that this will allow the student teacher to gain insight and confidence in the interviewing/hiring process. The school-based interview should occur after a minimum of 12 weeks into their student placement. If the student teacher has two, eight week placements, they should do their interview four weeks into their second placement. 

      For record-keeping, all I require is an email from the administrator saying that they have conducted the interview with the student teacher. This can be sent to me at jhaverland@dbq.edu.

      Leave of Absence Form URL

      This form must be completed by your student teacher and provided to the cooperating teacher, supervising teacher, and student teaching coordinator for any absence. Student teachers DO NOT have built-in days off for illness or any other personal business. If they exceed two absences, their placement may need to be extended. 

  • Initial meeting for cooperating and supervising teachers to meet with student teacher to plan placement.

  • First week of school

  • Formal observation 1 with meeting and feedback.

  • Formal observation 2 with meeting and feedback.

  • Progress Report 1 Due from Cooperating and Supervising Teachers (either individually or collaboratively).

  • Formal observation 3 with meeting and feedback.

  • Formal observation 4 with meeting and feedback.


  • Progress Report 2 Due from Cooperating and Supervising Teachers (either individually or collaboratively).

    • If an 8-week placement is ending, please complete the Formal Online Evaluation (individually or collaboratively) and finalize grade for placement.
    • If you are working with a 16-week placement, there  is nothing due at this time.

  • Formal observation 5 with meeting and feedback.

  • Formal observation 6 with meeting and feedback.

  • Progress Report 3 Due from Cooperating and Supervising Teachers (either individually or collaboratively).

  • Formal observation 7 with meeting and feedback.

  • Formal observation 8 with meeting and feedback.

  • Progress Report 4 Due from Cooperating and Supervising Teachers (either individually or collaboratively).
